The colon becomes markedly enlarged and its ability to move contents normally can be impaired.
In severe cases, complications can include perforation of the colon, severe infection, dehydration, electrolyte abnormalities, and shock.
That is why symptoms suggesting severe colonic inflammation or obstruction should not be managed casually at home.

Severe Constipation Alone Does Not Mean You Have Toxic Megacolon
This distinction is extremely important.
Someone who hasn't had a bowel movement for several days does not automatically have toxic megacolon.
The two conditions should not be treated as interchangeable.
Severe constipation can occur for many reasons, while toxic megacolon is a specific and uncommon complication usually associated with significant inflammation or infection of the colon.
If someone has severe constipation together with intense abdominal distension, severe pain, fever, rapid heart rate, vomiting, bloody diarrhea, or a generally worsening condition, urgent medical assessment is appropriate.
Warning Sign #1: Severe or Increasing Abdominal Pain
Mild cramping can occur with constipation.
But severe, persistent, or progressively worsening abdominal pain deserves attention.
Particularly concerning is pain accompanied by:

A swollen or rigid abdomen

Vomiting

Fever
-
Inability to pass gas

Significant tenderness

Rapid deterioration
These symptoms can occur with bowel obstruction or other serious abdominal conditions.
Don't assume that severe pain is simply the result of being “backed up.”
