Hypertension and Medication

Hypertension is a common chronic condition, especially among middle-aged and elderly people. Most patients need to take blood pressure–lowering medications regularly to keep their readings within a safe range, reducing the risk of cardiovascular complications and stroke. In addition to medication, patients must also adjust their diet and avoid foods that are harmful to their health.

In particular, after taking blood pressure medication, patients should avoid the following foods:

1. Fermented Tofu

Fermented foods like fermented tofu are often very high in salt.

  • Salt content: 100 grams of fermented tofu can contain 6–8 grams of salt—nearly double the maximum daily intake recommended by the World Health Organization (5 grams/day for adults).

  • Other risks: The fermentation process also produces nitrites, which are harmful to cardiovascular health.

Excess salt causes sodium ions to build up in the blood, disturbing the water–electrolyte balance. This increases blood volume and puts pressure on the vessel walls, making it harder for medication to control blood pressure. As a result, blood pressure may not remain stable or safe.

2. Grapefruit

Grapefruit is rich in vitamins and normally very healthy. However, when combined with blood pressure medications, it can be harmful.

  • Why? Grapefruit contains furanocoumarins, which inhibit the enzyme CYP3A4—responsible for breaking down drugs in the liver.

  • Consequence: When this enzyme is blocked, antihypertensive drugs cannot be metabolized properly. Their concentration in the blood rises abnormally, which may cause blood pressure to drop too low, leading to dizziness, fainting, or even drug shock.

👉 Patients taking antihypertensive medication should avoid eating grapefruit or drinking grapefruit juice right after taking their pills.

3. Alcohol

Alcohol is harmful to the body in many ways and should not be consumed with medication.

  • Interaction: Alcohol stimulates the renin–angiotensin system, causing blood vessels to constrict, raising blood pressure, and reducing the effectiveness of medication.

  • Greater risk with diuretics: For patients on diuretics, alcohol worsens dehydration. Since diuretics increase fluid excretion, combining them with alcohol may cause dizziness, fatigue, and sudden drops in blood pressure. Severe cases can lead to arrhythmia and stroke.

Recommendations for Hypertension Patients

  • Take medication consistently and maintain a healthy diet and lifestyle.

  • Diet tips:

    • Eat more fresh vegetables and fruits to increase fiber and potassium, which help stabilize blood pressure.

    • Choose whole grains such as corn, oats, and brown rice to support weight and blood sugar control.

    • Include protein-rich foods such as eggs, fish, lean meat, and soy products to maintain muscle mass and protect heart health.

  • Lifestyle: Exercise moderately and maintain regular daily routines to better control blood pressure.
