Chapter 2: Recognizing the Symptoms and Seeking Diagnosis

One reason PMR can be difficult to identify is that its symptoms overlap with several other disorders. Arthritis, fibromyalgia, thyroid conditions, and certain autoimmune diseases may produce similar complaints.

The most common symptoms include:

• Pain and stiffness in both shoulders.
• Neck discomfort and reduced mobility.
• Aching in the upper arms, hips, thighs, or buttocks.
• Morning stiffness lasting more than half an hour.
• Fatigue and reduced energy levels.
• Mild fever and general feelings of illness.
• Difficulty performing everyday tasks.

Doctors usually diagnose PMR based on a combination of symptoms, physical examination findings, blood tests, and imaging studies when necessary. Blood tests often reveal elevated markers of inflammation, such as erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP).

An important consideration is the connection between PMR and giant cell arteritis (GCA), another inflammatory condition affecting blood vessels. Some people with PMR may develop GCA, which can cause headaches, jaw pain, scalp tenderness, or vision problems. Immediate medical attention is required if these symptoms occur.
